Medical Services
The SWC includes a confidential and welcoming medical practice that specializes in the care of college-aged individuals.
Counseling Services
F&M counseling services work with students experiencing a wide variety of concerns in a confidential and welcoming atmosphere. Our diverse staff of licensed clinicians are experienced with and adept at addressing a broad range of challenges including depression, anxiety, homesickness, interpersonal problems, grief, eating concerns, and academic stress. If we cannot support your specific needs, we'll help connect you to a specialist in the community who can.

MyLGHealth is an online health portal that provides you with access to your medical
records. See below for instructions to create your account without an activation code.
If you would prefer to sign up with an activation code, please call the Student Wellness

Health insurance coverage is required for all full-time students. The Student Wellness
Center at F&M is part of Lancaster General Health system. Services at the Student
Wellness Center are typically billed to your insurance. See the SAHF/Student Administrative Health Fee document distributed by F&M for possible exceptions. Please contact your health insurance
provider to check that your insurance covers care provided in Lancaster, PA. Tami Edgell, Front Office Supervisor QM® Student Health Insurance Administrators & Advocates New students need to fill out a health history form indicating personal and family
health history, demographic information, tuberculosis assessment, and immunization
records, including meningitis vaccine requirements. Completion of this form is mandatory
for all new and all transfer students. Please note: Pennsylvania State Law requires that all college students residing in
a dormitory must either receive the meningitis vaccine (quadrivalent vaccine is the
CDC and F&M recommended vaccine), or sign a waiver declining the vaccine for “religious
or other reasons.” You will not be able to receive your room key on move-in day without
submitting proof of the vaccine or the signed waiver.Useful Resources

Health Insurance
You must enroll in F&M’s student health insurance program if you don’t have insurance
or if your insurance does not provide comparable coverage — including mental health
— to the school sponsored plan.
All F&M students receive six free counseling service visits prior to billing insurance

Incoming Student Health Forms
This form will be available in your Class of 2029 Canvas course. You must log into Canvas using your F&M Gmail. Please follow the instructions on completion and submission of the required forms.
You can direct all questions related to health form requirements to studentwellness@fandm.edu.
Once the Student Wellness Center (SWC) has received and processed your forms, they will mark them as complete in your Class
of 2029 Canvas course. If the SWC needs additional information, they will let you know. If this is the
case, you only need to send what they’ve requested, not the full set of forms again.
If you submit your forms and don’t receive a response, it just means the SWC hasn’t
yet reviewed your forms; there’s no need to resubmit.
